Information for Labs
Reporting Cancer Cases to Utah Cancer Registry
Cancer is designated as a reportable disease in the state of Utah (Cancer Reporting Rule, R384-100).
Laboratories that identify reportable conditions must submit reports about those conditions to the Utah Cancer Registry electronically. Laboratories should submit reportable case information when 1) patients are Utah residents and/or 2) when the ordering provider is located in Utah, regardless of patient residence. We will work with each lab to determine the best reporting mechanism. Contact us for more information.
What to Report
We prefer electronic pathology reports in HL7 format following the NAACCR Volume V standard. Laboratories may use National Program of Cancer Registries’ lists of reportable ICD-10-CM codes to identify reportable conditions, these lists are updated annually. Each report should include the following information:
- Laboratory and ordering provider information
- Patient’s demographic information
- Name (first, middle, last), date of birth, sex, race, ethnicity, state of residence. If available: social security number, full address, and phone number
- Diagnosis
- Results of cancer-directed laboratory examination, assays, and tests
How to Report
Pathology reports must be submitted electronically. We will work with you to determine the best submission format, method, and schedule for your laboratory. If you do not currently submit cancer reports to us or would like to discuss if another submission method would work better, please contact us.
The data received from entities required to report under the Cancer Reporting Rule (R384-100) is included in Record Series #31597.
